Job summary

VIA Health Partners is seeking an experienced Registered Nurse (RN) in Lincolnton, NC to deliver and coordinate hospice and palliative care for patients and families. You will perform nursing assessments, develop and update care plans, and educate families to support independence and comfort.

The role requires an active RN license, CPR certification, and at least 1 year of inpatient or hospice experience. Bachelor’s degree is preferred, and CHPN is a plus.

Qualifications

• A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.
• A current, active, and unrestricted multi-state RN license is required.
• Current CPR certification required; CHPN preferred.
• Minimum 1 year related inpatient experience; hospice/palliative care experience preferred.

Responsibilities

• Coordinate and deliver patient/caregiver care.
• Perform nursing assessments to guide patient/caregiver management.
• Develop and update care plans to ensure uninterrupted care and achievement of goals.
• Educate patients and families; enable caregiver independence where possible.
• Communicate with the IDG and other health professionals to ensure continuity of care.
• Maintain timely clinical documentation and ensure regulatory compliance.
