Job summary

VIA Health Partners in North Carolina is seeking a Registered Nurse to join our hospice and palliative care team. You will coordinate and deliver patient care, conduct nursing assessments, and educate patients and families while maintaining compliance with infection control and licensure standards.

The role requires an active multistate RN license, CPR, and experience in inpatient or hospice settings. We offer comprehensive benefits, retirement contributions, and professional development

Qualifications

• A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.
• Current, active, and unrestricted multistate license as an RN is required.
• Current CPR required; CHPN preferred.
• Minimum 1-year related inpatient experience; 2 years for other nursing positions.
• Computer proficiency is required.

Responsibilities

• Delivery of patient care coordination and assessment.
• Coordinate and update plan of care to ensure uninterrupted care.
• Educate patients and families; support caregiver independence.
• Communicate effectively with IDG, patients, and families for continuity of care.
• Maintain infection control and adherence to professional guidelines.
