Job summary

VIA Health Partners is seeking an RN to admit patients to hospice services in home or community settings, developing individualized plans with the interdisciplinary team. You will be the initial RN member driving care goals and coordinate with physicians, staff, and external providers to ensure timely service delivery.

The role requires a nursing degree and active RN license in NC/SC, CPR, and experience in hospice or palliative care.

Qualifications

• Diploma or ADN required; BSN preferred.
• Current NC/SC RN license.
• CPR certification required; CHPN preferred.
• Minimum 2 years direct patient care experience in hospice/palliative or related fields.
• Pediatric hospice experience (Kids Path) preferred.
• Proficiency with electronic medical records.

Responsibilities

• Present hospice services at admission and coordinate initial care plan.
• Conduct admission nursing assessment and develop plan of care.
• Coordinate with interdisciplinary team and communicate with staff.
• Provide patient and caregiver education at admission.
• Ensure timely clinical documentation and medication records.
• Support Nursing Assistant care planning and team meetings.
• Maintain professional relationships with external providers.
