Job summary

VIA Health Partners invites applications for an RN hospice nurse in North Carolina. The role focuses on coordinating patient/caregiver care, performing nursing assessments, and adjusting care plans to ensure uninterrupted, goal-driven care.

You will educate families and collaborate with the IDG and other health professionals to ensure seamless continuity of care. The position requires active RN licensure, CPR, and a commitment to infection control.

Qualifications

• Diploma or Associate Degree in Nursing required; Bachelor’s degree preferred.
• Current, active RN license (multistate is allowed) in home state; NC license required for practice.
• Current CPR certification required.
• CHPN certification preferred.

Responsibilities

• Coordinates and delivers patient/caregiver care.
• Conducts nursing assessments to guide care.
• Develops/updates plan of care to ensure uninterrupted care.
• Educates patients and families and answers questions.
• Communicates with IDG and other health professionals for continuity of care.
• Maintains infection control and follows universal precautions.
