Job summary

Gentiva Hospice is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends and holidays, delivering compassionate care when it’s most needed. You’ll report to the Administrator or Patient Care Manager Senior and coordinate with the hospice team to manage symptoms and plans of care.

The role requires an active RN license, 1+ year nursing experience, CPR certification, and a valid driver's license.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year of nursing experience.
• CPR certification is required.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to patient needs during evenings, nights, weekends and holidays.
• Provide direct patient care and support after normal business hours.
• Serve as liaison between patients, families, physicians and the hospice team.
• Document clinical visits and communications accurately.
• Collaborate with interdisciplinary team to meet Plan of Care.
• Participate in quality assessment and performance improvement programs.
• Maintain continuity of care and coordinate with daytime care team.
