Qualifications

• Ability to operate computerized systems to enter/modify patient information
• Ability to schedule medical appointments and manage patient support tasks
• Ability to communicate effectively with internal and external customers
• Familiarity with interdisciplinary care delivery concepts and community care processes

Responsibilities

• Coordinate with patient care team to monitor clinic utilization and referrals
• Communicate with patients, interdisciplinary teams, and VA centers
• Process secure messaging and manage follow-ups for consults and tests
• Maintain tracking systems for follow-up care and scheduling
• Assist in training and mentoring others on processes and tools
• Review appointment availability across EPIC/HSRM and community care
• Ensure medical documentation is scanned and uploaded for timely closure
