Human Services Administration Organization (HSAO) provides person-centered service coordination that helps children, teens, and adults with serious mental illness live healthier, more independent lives. We believe recovery is possible. Our service coordinators partner with clients to build on their strengths, overcome barriers, and connect to meaningful community supports.

Job Overview:

The Adult Blended Service Coordinator serves as the primary point of accountability for adults receiving mental health services. This community-based role combines assessment, advocacy, outreach, service planning, and care coordination to help individuals achieve their recovery goals while addressing social determinants of health. This role is ideal for a self-starter who excels in a collaborative environment and demonstrates initiative, empathy, and strong communication skills.

Program Specific Description:

Adult Blended Service Coordinator serves adults (18+) throughout Allegheny County, including home, treatment locations, hospital, court, etc. Service Coordinator complete comprehensive assessments, service plan, and crisis plan. Service Coordinator links treatment services, resolves problems, and connects to community resources. Frequency of face-to-face contact with clients is determined by assessment – at least every 30 days. Clients qualify based on having a current mental health diagnosis, actively participating with service coordinator, and committing to utilizing treatment services. Typical service coordination caseload is 25-30 clients, depending on intensity of needs.

Key Responsibilities:

Client Support and Case Management

• Serve as the Single Point of Accountability (SPA) for clients, acting as the primary contact and advocate for services.
• Build and foster enduring relationships with clients through persistent outreach and engagement.
• Develop targeted, measurable, and achievable service plan goals tailored to individual strengths and needs, regularly updating and refining as necessary.
• Maintain monthly face-to-face contact with clients, ensuring services are provided in line with program standards.
• Provide culturally competent services with consideration for the individual’s racial, religious, sex, sexual orientation, age, and ethnic background and identification.

Collaboration and Advocacy

• Coordinate care across various providers, including: behavioral health, substance use services, criminal justice system, educational/vocational supports, community resources, housing programs, Office of Developmental Supports, Area Agency on Aging, etc.
• Facilitate interdisciplinary team meetings and planning, providing follow-up and ensuring continuity of care.
• Act as an advocate for clients, helping remove systemic barriers, resolving issues, and fostering full inclusion within their community.
• Support acquisition and renewal of qualifying government benefits and entitlements.
• Partner with natural supports to accomplish service plan goals.

Documentation and Compliance

• Complete assessments, service plans, and case notes within specified timeframes, ensuring all documentation complies with state, managed care, and agency regulations.
• Maintain accurate records of all interactions, including meetings, service linkages, and follow-up activities in electronic health records.
• Comply with HIPAA confidentiality policies and participate in regular audits and program evaluations.

Professional Development

• Participate in ongoing training and development opportunities to stay updated on best practices and trends in mental health, justice, benefits, service coordination, etc.
• Actively utilize individual and group supervision to triage clients, reflect on engagement, and share resources.

Required Skills and Qualifications:

• Bachelor’s degree in Social Work, Psychology, Education, Criminal Justice, or a related field (Master’s degree welcome).
• 2 years of professional experience working with adults in mental health, criminal justice, education, vocational, housing, or social service setting. Strong organizational skills with the ability to manage multiple priorities and meet deadlines.
• Excellent communication and interpersonal skills, with the ability to work independently and as part of a multidisciplinary team.
• Demonstrated cultural competence and the ability to work with diverse populations.
• Successful service coordinators build trusting relationships, maintain timely documentation, meet utilization standards, collaborate effectively with providers, and help individuals achieve meaningful recovery goals.

Additional Requirements:

• Valid Pennsylvania driver’s license with access to a reliable vehicle.
• Current auto insurance that meets minimum collision coverage requirements.
• Act 33, Act 34, and FBI Clearances (including Institutional Clearances if working with incarcerated individuals).
• Ability to complete required onboarding training within 30 days of hire.
• Participation in on-call rotations as needed.

Work Conditions and Benefits:

• Full-time position (40 hours per week), with weekly evening hours and rare weekend hours to meet client needs.
