The Service Coordinator (SC) is responsible for locating, coordinating, and monitoring supports and services that assist adults with disabilities in their homes and community settings. The SC is often the initial contact for clients, so strong people skills and customer service are mandatory. Building relationships with partners and service providers ensures clients receive the best care possible.
The shift is 8:00 am to 4:30 pm, Monday through Friday, and the role is hybrid. Work is performed at home and requires in-person visits to clients at their residences. The candidate must be willing to travel using their own vehicle within the assigned territory, with mileage reimbursement available, and must travel throughout Allegheny, Westmoreland, Fayette, Indiana, Armstrong, Butler and Beaver counties, with the majority of cases in Allegheny and Westmoreland.
