This role involves assessment, service planning, advocacy, and connecting clients with community resources; schedule Monday–Friday 8:30am–5:00pm with on-call rotations. A sign-on bonus of $3,000 is offered.

Are you an individual with a bachelor’s degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education? UPMC Western Behavioral Health at Safe Harbor is seeking a passionate Service Coordinator I to support our vibrant Blended Case Management team!
Blended Case Management (BCM) is a supportive, individualized service that helps children, adolescents, and adults with mental health or behavioral challenges access community resources, coordinate care, and achieve personal recovery goals.
As a Service Coordinator I, you will provide comprehensive case management services that focus on assessment and service planning, care coordination, advocacy, monitoring and evaluation, crisis and safety planning, education and support, and connecting individuals to community programs, resources, and services as needed. Working collaboratively with individuals and their support systems, you will help identify needs, remove barriers, and address social determinants of health through a recovery-oriented approach that integrates mental health and overall health care. Your efforts will center on developing targeted, measurable, and achievable goals that promote stability, independence, and success while empowering individuals to believe that recovery is possible and within reach for everyone.
This position is eligible for the Erie County Human Services Cohorts (HSC) Initiative! This program is focused on growing and retaining the workforce for local agencies addressing mental health, substance use disorders, and social drivers of health. Designed to strengthen the county’s behavioral health system, this program offers incredible benefits, including up to $45,000 in educational loan repayment, a $5,000 stipend and access to cohort sessions for skill-building and networking!
