Job summary

Chartiers Center in Pittsburgh is seeking a Blended Service Coordinator to support adults with serious mental illness and co-occurring conditions in community settings.

You will assess needs, develop individualized service plans, connect clients with housing, benefits, healthcare and community resources, and coordinate with providers, families and agencies to promote recovery and independence.

Qualifications

• Associates degree in human services with 3+ years mental health experience.
• Bachelor’s degree in human services with 2+ years mental health experience.
• Master’s degree in human services with 1 year of experience.
• Valid driver's license required for community travel.
• Ability to travel throughout the community.

Responsibilities

• Meet individuals in homes and community settings.
• Conduct assessments to identify strengths, needs and goals.
• Develop, implement and monitor individualized service plans.
• Connect clients with housing, benefits, healthcare, behavioral health treatment, employment and community resources.
• Coordinate services among healthcare providers, therapists, families and community agencies.
• Maintain regular communication to monitor progress and emerging needs.
• Advocate across multiple systems on behalf of individuals.
• Maintain timely and accurate documentation.
• Participate in an on-call rotation (typically no more than once per month).
