Water’s Edge Rehab & Nursing Center in Port Jefferson, NY seeks an experienced Assistant Director of Nursing (ADON) – Risk Management & Education to join the clinical leadership team. This RN leader will enhance resident safety, drive staff development, and uphold regulatory standards.

Responsibilities include risk management, QA/QI initiatives, staff coaching, and cross-functional collaboration with Nursing, Infection Prevention, and interdisciplinary teams to ensure high-quality resident care.

Qualifications

• Current and active New York State RN license.
• BSN preferred.
• Experience in ADON, Nurse Manager, Staff Development or nursing leadership.
• Skilled Nursing Facility, Long-Term Care, or Rehabilitation experience.
• Strong knowledge of CMS and New York DOH regulations.
• Experience with risk management, incident investigation, QAPI, and audits.
• Strong staff education and presentation skills.
• Excellent communication, leadership, organizational and critical-thinking abilities.
• Ability to identify clinical risks and implement solutions.
• Resident-centered care and quality outcomes commitment.

Responsibilities

• Assist the Director of Nursing with daily clinical operations.
• Oversee facility-wide risk management and resident safety programs.
• Review and investigate incidents, accidents and adverse events.
• Identify trends and develop corrective action plans.
• Monitor quality indicators (falls, injuries, infections, hospitalizations).
• Assist with QAPI initiatives and performance improvement activities.
• Conduct regular clinical audits and identify improvement opportunities.
• Develop, coordinate, and deliver nursing education and competency programs.
• Provide orientation and ongoing education to nurses and CNAs.
• Ensure staff education aligns with regulatory requirements and policies.
• Maintain records of employee education and competencies.
• Collaborate with interdisciplinary teams for quality care.
• Review clinical documentation for compliance and quality.
• Assist with state and federal surveys and readiness.
• Ensure CMS and NYSDOH regulatory compliance.
• Provide clinical guidance and mentorship to nursing staff.
• Develop and update nursing policies and procedures.
• Support leadership with staff performance and competency concerns.
