Job summary

Apploi is seeking an experienced Assistant Director of Nursing (ADON) – Risk Management for a full-time, in-person role in New York. The ADON will lead risk management, oversee resident safety, and support regulatory compliance across the facility.

Qualifications

• RN license active in the applicable state.
• BSN preferred.
• Prior ADON/Assistant DON/Nurse Manager or risk leadership experience preferred.
• Skilled Nursing Facility, Long-Term Care, or Rehabilitation experience preferred.
• Strong knowledge of CMS regulations, state requirements, quality measures, and survey processes.
• Experience with incident investigation, risk management, QAPI, and clinical audits.
• Strong leadership, communication, organizational and critical-thinking skills.
• Ability to identify problems, develop solutions, and follow through.

Responsibilities

• Assist the Director of Nursing with overall clinical operations of the nursing department.
• Lead and oversee the facility's risk management and resident safety programs.
• Monitor incidents, adverse events, and potential areas of clinical risk.
• Conduct investigations and develop corrective action plans.
• Identify trends and implement strategies to prevent recurring incidents.
• Assist with QAPI initiatives and clinical documentation reviews.
• Monitor falls, infections, medication errors, and other quality indicators.
• Collaborate with nursing leadership, physicians, therapy, and social services.
