You will guide clients through admissions, respond to referrals, conduct screenings, and coordinate with clinical, care management, and billing teams to ensure a smooth transition into services. You will maintain accurate records, manage databases, operate phone systems, and support census management while upholding privacy and ethical standards.

Qualifications

• Must have a high school diploma or GED.
• Strong verbal and written communication skills are essential.
• Ability to prioritize tasks, manage multiple responsibilities, and maintain attention to detail.

Responsibilities

• Create a welcoming admissions experience by building rapport and delivering excellent customer service.
• Guide clients through telephone or in-person screenings and document referral information.
• Complete admissions in applicable databases and manage residency and financial information.
• Coordinate with Care Managers to verify insurance details and support transition into services.
• Collaborate with Billing to ensure admissions and transfers meet required timeframes.
