Job summary

Empire Care Centers in Georgia is seeking an experienced Central Intake/Admissions Coordinator to streamline the admissions process and support census growth across our long-term care facilities.

The role requires prior admissions experience in skilled nursing or rehab, familiarity with PCC and Microsoft Office, and the ability to review referrals, assess clinical appropriateness, and prepare admission documentation.

Qualifications

• Prior experience in an admissions coordinator, director or marketing position in a skilled nursing facility or rehab center is a must.
• Associates degree or higher preferred.
• Solid computer skills, including Microsoft Office products and PCC.
• Working knowledge of federal & state regulations and reimbursement (Managed Care, Medicare, and HMOs).
• Working knowledge of medications and diagnoses with the ability to review medical patient information.

Responsibilities

• Coordinate the central intake / admissions process and support census growth.
• Work closely with external marketing and admission liaisons.
• Provide prospective residents and their families with facility and program information.
• Review incoming referrals for acceptance.
• Evaluate and assess potential patients for clinical appropriateness and placements.
• Help to ensure proper admission documentation at the building level.
• Participate in facility sales and marketing events and plans as needed.
