Qualifications

• Experience in nursing home admissions or similar healthcare role.
• Proven ability to build and maintain relationships with referral sources.
• Willingness to drive to hospitals, physician offices, and other referral partner locations.
• Experience running financials for referrals: payer review, insurance screening, and benefit verification.
• Strong multitasking ability to manage tours, follow-up, admissions coordination, and census tasks.
• Excellent verbal and written communication skills.
• Friendly, outgoing, and professional demeanor.
• Strong organizational skills and attention to detail.
• Proficiency with email, spreadsheets, admissions logs, and CRM systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Support nursing home census growth and deliver a superior experience for residents and referrals.
• Develop and maintain referral relationships with hospitals, rehab centers, physicians, and other partners.
• Travel regularly to referral sources and partner locations to strengthen relationships.
• Coordinate the admissions process from inquiry to move-in.
• Conduct resident and family tours with warmth and professionalism and follow up.
• Review referrals' financials, payer sources, insurance verification, and admission opportunities.
• Collaborate with clinical, business office, and leadership teams for smooth transitions.
• Maintain consistent communication with families, residents, hospital teams, and internal departments.
• Track referrals, conversions, denials, and census-related metrics.
• Support census improvement strategies and outreach plans.
• Document referral activity, admissions status, and follow-up actions accurately.
• Represent AMA at community events, networking opportunities, and referral meetings.
