Job summary

iCare Consulting Services seeks an Intake Coordinator Long Term Care to manage admissions from home, hospital, or other health care settings, and to handle Medicare, Medicaid, and private billing and collections. You will ensure capacity and complete admissions documentation for each resident.

The role requires 2+ years in long-term care, proficiency with CarePort and Microsoft Office, and strong organizational skills to support discharge planning and referral satisfaction.

Qualifications

• 2+ years work experience in Long Term Healthcare and insurance verification.
• Proficient with CarePort software application.
• Proficient with Microsoft Office applications.
• Proficiency in basic math required.
• Functional literacy in English is required, ability to understand and follow oral and written directions.

Responsibilities

• Receive all referral inquiries regarding prospective residents to determine admission date or schedule an appointment.
• Inform necessary departments of arrival of new residents and coordinate admission per facility policy.
• Verifies insurance benefits with carriers.
• Complete admissions packet and necessary documentation for every admission.
• Maintain relationships with hospital social workers, discharge planners, and case managers to stimulate referrals and monitor satisfaction.
