Job summary

Evergreen Healthcare Group is seeking an Admission Coordinator at The Dalles Health and Rehabilitation in The Dalles, Oregon. You will be the first point of contact for referrals and families, coordinating admission processes and ensuring a seamless transition into the facility.

Ideal candidates have SNF/rehab admission experience, strong organizational skills, and proficiency with PCC and healthcare portals. Join a team dedicated to compassionate care and operational excellence.

Qualifications

• Prior SNF, rehab, or healthcare admission experience strongly preferred.
• Detail-driven, organized, and able to manage multiple moving parts.
• Customer-focused with empathy, clarity, and professionalism.
• Tech-savvy with proficiency in care portals and common software tools.

Responsibilities

• Serve as the primary contact for all incoming referrals, calls, walk-ins, and inquiries.
• Coordinate admission documentation completion, insurance verification, and pre-admission authorization.
• Communicate clearly with hospitals, case managers, residents, families, and internal departments.
• Ensure accurate data entry and timely updates in Point Click Care CRM.
• Facilitate facility tours, referral follow-up, and participation in conversion strategies.
• Maintain strong customer service standards at every touchpoint.
• Collaborate with nursing, therapy, and administrative teams to ensure smooth admissions.
• Ensure empty rooms are admit and tour ready on a daily basis.
• Assist with marketing events, community outreach, and relationship development activities.

Job Summary

The Dalles Health and Rehabilitation in The Dalles, Oregon is looking for an Admission Coordinator for our community. As our Admission Coordinator, you will be the first point of communication for referrals, families, and partners. • Collaborative – A team player who works well with nursing, clinical, and leadership staff.
