The role requires experience in SNF or rehab admissions, excellent organization, and professional communication. Proficiency with PCC and various insurance portals helps support efficient admissions and service excellence.

Qualifications

• Experience in SNF, rehab, or healthcare admissions preferred.
• Highly organized with strong attention to detail.
• Empathetic, clear, professional communication.
• Proficient with PCC, insurance portals, and hospital referral portals.

Responsibilities

• Serve as the primary contact for referrals, calls, walk-ins, and inquiries.
• Coordinate admission documentation, insurance verification, and pre-admission authorization.
• Communicate clearly with hospitals, case managers, residents, families, and internal teams.
• Ensure accurate data entry and timely updates in PCC.
• Facilitate facility tours, referral follow-up, and conversion strategies.
• Maintain strong customer service at every touchpoint.
• Collaborate with nursing, therapy, and admin teams for smooth admissions.
• Ensure rooms are admit-ready daily.
• Assist with marketing events and community outreach.
