Job summary

SanStone Health in Hendersonville, NC, is seeking an Admissions and Social Services Assistant to join our team. The role involves interviewing patients, developing care plans, coordinating with families and discharge planners, and ensuring accurate admissions documentation for skilled nursing care.

The ideal candidate will have experience in a long-term care setting, excellent communication skills, and a customer-service mindset.

Qualifications

• Skilled Nursing Facility/Long Term Care experience preferred.
• Ability to deal tactfully with residents, families, staff and the public.
• Excellent written and verbal communication skills.
• On-call schedule required every other weekend

Responsibilities

• Assess nature of patients' situations by interviewing and reviewing personal history.
• Develop and execute individual treatment plans/care plans.
• Communicate patient updates to all relevant parties.
• Facilitate referrals to other healthcare professionals and programs as needed.
• Reach out to discharge planners, families and other referral sources.
• Serve as the initial liaison with residents and families on facility services.
• Obtain necessary personal and financial data to determine admission eligibility.
• Assign accommodations for new admissions based on needs and space availability.
• Ensure admissions paperwork is completed thoroughly and timely.
• Maintain records of admissions and discharges; participate in occupancy meetings.
• Maintain knowledge of Medicare & Medicaid regulations to ensure compliance.

Responsibilities

• Assess nature of patients' situations by interviewing and reviewing personal history
• Develop and execute individual treatment plans/care plans
• Communicate patient updates to all relevant parties
• Facilitate referrals to other healthcare professionals and programs as needed
• Proactively reach out to and responds to inquiries from hospital discharge planners, families, and other referral sources
• Serve as the initial liaison with residents and families on facility services
• Speak with residents or residents’ representative to obtain necessary personal and financial data to determine eligibility for admission
• Assign accommodations for new admissions related to nursing care needs, resident and residents’ family’s preference, availability of space, and other information.
• Ensure all admissions paperwork is completed thoroughly, accurately and on a timely basis.
• Maintain records of admissions and discharges and compile occupancy census data. Participate in meetings to alert appropriate staff members of projected admissions, room changes, and discharges
• Maintain knowledge of Medicare & Medicaid regulations to ensure services are in compliance with professional boarding standards, state and federal regulatory requirements.

Qualifications

• Skilled Nursing Facility/Long Term Care experience is preferred. Candidate must have the ability to deal tactfully with personnel, residents, visitors, and the general public. The ideal candidate should have a passion for working with seniors, have compassion, and a high degree of customer service and sales/marketing experience. Candidate must also possess excellent written and verbal communication skills. On-call schedule required every other weekend to assist with admissions.
