SanStone Health & Rehabilitation in Hendersonville, NC is seeking an Admissions and Social Services Assistant to join our dedicated health care team. You will assess patient situations, develop care plans, and coordinate admissions with families and hospital discharge planners.

Applicants should have a passion for working with seniors, strong communication skills, and the ability to handle admissions paperwork and data accurately in a supportive, team-oriented setting.

Qualifikationen

• Experience in Skilled Nursing Facility/Long Term Care preferred.
• Strong written and verbal communication skills.

Aufgaben

• Assess nature of patients' situations by interviewing and reviewing personal history
• Develop and execute individual treatment plans/care plans
• Communicate patient updates to all relevant parties
• Facilitate referrals to other healthcare professionals and programs as needed
• Proactively reach out to and responds to inquiries from hospital discharge planners, families, and other referral sources
• Serve as the initial liaison with residents and families on facility services
• Speak with residents or residents’ representative to obtain necessary personal and financial data to determine eligibility for admission
• Assign accommodations for new admissions related to nursing care needs, resident and residents’ family’s preference, availability of space, and other information.
• Ensure all admissions paperwork is completed thoroughly, accurately and on a timely basis.
• Maintain records of admissions and discharges and compile occupancy census data. Participate in meetings to alert appropriate staff members of projected admissions, room changes, and discharges
• Maintain knowledge of Medicare & Medicaid regulations to ensure services are in compliance with professional boarding standards, state and federal regulatory requirements.
