Job summary

Select Home Health & Hospice, part of Fortis Health, is seeking an Admission Registered Nurse to perform home health admissions, comprehensive assessments, and care planning. You will coordinate with physicians and the interdisciplinary team to ensure high-quality, personalized in-home care for multiple patients daily.

Responsibilities include developing physician-ordered plans of care, educating patients and caregivers, and ensuring compliance with all regulations and agency policies.

Qualifications

• Graduate of an accredited nursing program.
• Current Registered Nurse license in the practicing state.
• Minimum of 1 year nursing experience in hospital, facility, or home health setting; home health experience preferred.
• Valid driver’s license, reliable auto insurance, and personal transportation required.
• Current TB test up to date.

Responsibilities

• Complete home health admissions, comprehensive patient assessments, and ongoing evaluations.
• Develop and implement physician-ordered plans of care with accurate documentation (OASIS).
• Provide skilled in-home nursing care to multiple patients daily.
• Educate patients and caregivers on medications, disease processes, treatment plans, and home care needs.
• Coordinate patient services and supervise LPNs, HHAs, and other staff as appropriate.
• Communicate changes in patient condition to physicians and interdisciplinary care team.
• Ensure compliance with federal, state, and agency regulations, policies, and standards of care.
• Collaborate with the interdisciplinary team to promote positive patient outcomes and continuity of care.
