Job summary

Advocate Health seeks a licensed Speech-Language Pathologist for a part-time role in the Advocate Condell Medical Center - Acute Care Rehabilitation. The position requires evaluation of patients with speech, language, voice, cognitive and swallowing disorders and development of treatment plans.

Must work at least one weekend day per month and one holiday per year. Responsibilities include discharge planning, documentation, and collaboration with physicians and staff.

Qualifications

• Excellent communication, interpersonal, and organization skills.
• Intermediate computer skills and ability to document and use clinical systems.

Responsibilities

• Interprets physician referrals and conducts a speech and language evaluation.
• Establishes a written plan of care and implements treatment based on evaluation results.
• Plans for discharge including home programs and family teaching.
• Completes required documentation and monitors patient progress.
