Job summary

Advocate Health in Illinois is seeking a Speech Language Pathologist for a primarily weekend Acute Care PRN role between Advocate South Suburban and Advocate Trinity in the South Chicago area. The position involves 1-2 shifts per month with occasional weekday PRN availability.

You will evaluate patients, develop and implement care plans, document progress, and support discharge planning. Responsibilities include interprofessional collaboration, adherence to policies, and ongoing education.

Qualifications

• Master’s degree in Speech Pathology from an accredited school.
• CCC-SLP certification preferred.

Responsibilities

• Interprets physician referrals and conducts a speech and language evaluation of patients with various speech, language, voice, cognitive and swallowing disorders.
• Establishes a written plan of care and implements treatment based on evaluation results and goals.
• Plans for and completes patient discharge including home programs and family teaching.
• Documents care and progress, billing and scheduling, and adheres to policies and timelines.
• Promotes patient safety and collaborates with the care team; may supervise students.
