A leading acute care hospital in Central Florida is seeking a Director of Case Management to lead and grow the established Case Management team. The role emphasizes patient outcomes, care coordination, and operational excellence.

This position requires experience guiding change, staff development, and aligning case management with hospital strategy. The Director will oversee utilization review, discharge planning, and regulatory compliance while partnering with physicians and hospital leadership

Qualifications

• Bachelor's degree required.
• RN or LCSW licensure required (RN strongly preferred).
• Minimum of 2 years of recent acute care Case Management leadership experience (within the last year).
• Strong understanding of utilization review, discharge planning, care coordination, and regulatory compliance.
• Proven leadership and team development experience.

Responsabilités

• Lead and develop the Case Management department.
• Drive effective discharge planning and care coordination initiatives.
• Partner with physicians, nursing, and hospital leadership to improve patient outcomes and operational performance.
• Ensure compliance with organizational, regulatory, and accreditation standards.
• Monitor departmental metrics and implement process improvements to support quality and efficiency goals.
