Job summary

LeaderStat, LLC seeks an experienced Director of Case Management to lead operations at a 221-bed acute care hospital in Sanford, FL. The role emphasizes discharge planning, care coordination, clinical resource utilization, and performance improvement under hospital leadership.

You will supervise 13–15 staff, including a Case Management Manager and Lead, and partner with the CMO, CNO, and Medical Director to improve outcomes, ensure regulatory compliance, and drive departmental performance.

Qualifications

• Minimum 2 years of recent acute care Case Management leadership experience.
• Bachelor's degree required; RN or LCSW credential.
• Experience leading case management operations in acute care.
• Strong discharge planning, care coordination, regulatory compliance and performance improvement.

Responsibilities

• Provide strategic and operational leadership for the Case Management department.
• Oversee daily operations, staffing, productivity, performance metrics and quality initiatives.
• Lead care coordination, discharge planning and patient transition processes.
• Partner with CMO, CNO, Medical Director, and senior leadership to meet goals.
• Evaluate performance, identify improvement opportunities, implement solutions.
• Ensure compliance with policies, regulations, and case management standards.
• Manage staffing, onboarding, and performance expectations; develop the team.
• Build interdisciplinary relationships to support patient outcomes and resource use.
