Job summary

Healthcare facility seeks a Director of Case Management to lead care-management, utilization management, discharge planning, social work, and psychosocial interventions. Role focuses on safe, effective transitions of care, patient outcomes, satisfaction, throughput, and alignment with benchmarks.

Strategic and operational plans, along with management of clinical, operational, and financial metrics, are key parts of the position.

Qualifications

• Bachelor's in nursing and Master's in health-related field required.
• 2+ years of care management in an acute-care setting.
• 2+ years of healthcare leadership related to acute-care case management.
• Strong knowledge of acute-care case management principles.
• Understanding of evidence-based practice and care-transition models.
• Knowledge of the care continuum and transition-of-care strategies.

Responsibilities

• Provide strategic and operational leadership for care-management functions across the facility.
• Accountable for patient outcomes, satisfaction, throughput, and care continuum utilization.
• Develops and executes strategic and operational plans and manages key metrics.
• Ensures services are patient-centered and aligned with benchmarks.
• Drives performance to meet regional and national standards.
