Encompass Health Corporation is seeking a Case Management Director to lead the department, oversee daily operations, and guide the interdisciplinary care team. This leadership role focuses on improving patient outcomes, census management, and policy adherence across the organization.

The candidate should bring CCM/ACMTM certification (preferred), a nursing BSN with RN licensure or an equivalent bachelor’s in healthcare, and 3+ years of hospital-based Case Management experience, with willingness

Formación

• CCM or ACMTM certification preferred.
• Active license if required for the discipline.
• Nursing: BSN with RN licensure.
• Other eligible health care professionals: bachelor's degree; graduate degree preferred.
• Three years hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• May be required to work varied shifts.

Responsabilidades

• Oversee day-to-day operations of the Case Management department.
• Supervise discharge planning and plan of care to drive census management and outcomes.
• Advocate for patients and families; ensure services meet needs.
• Provide guidance to Case Managers; train on caseloads, policies, and procedures.
• Build relationships with insurers, self-insured employers, and health networks.
• Celebrate and recognize team achievements.
