Job summary

Delaware Valley Community Health is seeking an Access Nurse at Maria de los Santos Health Center in Philadelphia, PA. The role supports timely clinical communication between patients and care teams, handling MyChart messages, refill requests, and test result communication within LPN scope.

Based in a busy ambulatory care setting, you will triage patients, document in the EHR, and escalate issues to the Triage Nurse or provider as needed. Monday–Friday schedule, no weekends.

Qualifications

• Current and unrestricted Pennsylvania LPN license.
• Graduate of an accredited practical nursing program.
• Current CPR/BLS certification.
• Strong clinical communication, documentation, and organizational skills.
• Ability to recognize changes in patient condition and escalate appropriately.
• Ability to work efficiently in a high‑volume, team‑based environment.
• Ability to manage telephone, electronic, and EHR‑based patient communications.

Responsibilities

• Interviews/triages patients and/or family to determine need for medical attention.
• Collects and interprets patient data with a provider for initial evaluation or follow‑up.
• Records all patient data in the EHR after each encounter.
• Initiates diagnostic and therapeutic measures per standing protocols.
• Provides health counselling and instructions regarding common patient problems.
• Works with Clinical Pharmacist to support patient education on medications and adherence.
