Henrietta Johnson Medical Center is seeking a Triage Nurse to support value-based care by assessing patient needs in real time and directing to appropriate care levels. The on-site, non-exempt role focuses on safe, efficient triage and care coordination in Delaware.

With 3+ years of nursing experience and an active Delaware RN license, you will assess, educate, and refer patients, document actions in the EHR, and participate in quality initiatives to improve access and safety.

Qualifications

• Current unrestricted RN license in Delaware.
• Minimum 3 years nursing experience in primary/ambulatory/urgent care or telephonic triage.
• Proficient in clinical assessment, escalation, and documentation.
• Experience with electronic health records.

Responsibilities

• Perform telephone and in-person triage using clinical protocols and nursing judgment to determine urgency and disposition.
• Assess symptoms, risk factors, med issues, and red flags; escalate time-sensitive cases as needed.
• Coordinate timely appointments, referrals, testing, and follow-up per provider direction.
• Document triage assessments and care coordination steps accurately in the EHR.
• Support chronic disease and high-risk patient management by identifying deterioration and notifying the team.
• Provide patient education on self-care, red-flag symptoms, and when to seek urgent care.
• Participate in team huddles, process improvements, and quality initiatives for access and safety.
• Maintain licensure, competencies, infection control practices, and privacy compliance.

About This Role

The Triage Nurse supports value-based care by assessing patient needs in real time and directing patients to the most appropriate level of care. The role improves patient safety, timely access, and cost efficiency by reducing unnecessary emergency department utilization, prioritizing urgent needs, and supporting same-day or next-best care pathways.

Note: This is a Non-Exempt position, and the candidate must be able to work on-site.

What You’ll Do

• Performs telephone and in-person triage using approved clinical protocols, nursing judgment, and standing workflows to determine urgency and disposition.
• Assesses symptoms, risk factors, medication issues, and red-flag conditions and escalates emergent or time-sensitive situations to the appropriate clinician or emergency response pathway.
• Coordinates timely appointments, referrals, testing, and follow-up based on patient need and provider direction.
• Documents all triage assessments, advice provided, escalation actions, and care coordination steps accurately and timely in the electronic health record.
• Supports chronic disease and high-risk patient management by identifying worsening conditions and notifying the care team when intervention is needed.
• Provides patient education related to self-care, red-flag symptoms, medication adherence, and when to seek emergency or urgent care.
• Participates in team huddles, workflow improvement, and quality initiatives related to access, continuity, and patient safety.
• Maintains licensure, clinical competencies, infection control practices, and privacy and security compliance.

What You Bring

• Current unrestricted Registered Nurse license in the State of Delaware, or compact state authorization as applicable.
• At least three years of nursing experience, preferably in primary care, ambulatory care, urgent care, or telephonic triage.
• Strong clinical assessment, escalation, and documentation skills.
• Proficiency with electronic health record documentation and standard office technology.

Preferred Qualifications

• Experience in an FQHC or ambulatory care setting.
• Experience with evidence-based triage protocols and care management workflows.
• BLS certification and demonstrated comfort in fast-paced clinical operations.

This Role Is Grant-Funded

This position is funded through Delaware's federal Rural Health Transformation Program grant, awarded through the Delaware Department of Health and Social Services. Although the RHT award covers a five-year project period, funding is approved and allocated annually, and continued employment in this role is contingent on annual grant reapproval.
