Job summary

Sweetwater Care seeks a California-licensed LVN to provide nursing care within practice scope and meet patient needs per standards, orders, and guidelines. You will participate in quality improvement and educate families, while coordinating care with other departments.

The role requires an LVN in California (compact license if applicable), long-term care background, and PointClickCare charting experience; prior nursing management is preferred.

Qualifications

• Currently licensed as LVN in California.
• Compact license if working in a Compact state.
• 1 year nursing management experience preferred.
• Long-term care experience required.
• Experience with PointClickCare charting.

Responsibilities

• Supports infection control and quality improvement initiatives.
• Educates patients and families on care plans.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
