Job summary

Sweetwater Care in California is seeking an LVN with management experience to oversee nursing care within licensed scope and to support patient needs in alignment with center policies and physician orders.

The role involves participating in quality improvement, patient education, and timely administration of medications and treatments, while coordinating with physicians and other departments as changes occur in patient status.

Qualifications

• Licensed LVN in CA required; compact license acceptable in related states.
• At least 1 year of nursing management experience preferred.
• Experience in LONG TERM CARE setting is required.

Responsibilities

• Provide nursing care within scope of practice per standards, orders, and policies.
• Implement Quality Improvement Program and participate in related activities.
• Use Quality Life Manual to identify, place, and monitor patients in programs.
• Notify departments of patient changes (room, LOA, new admits, discharge).
• Educate patients and families.
• Administer medications and treatments timely per facility policy.
• Receive, transcribe, and execute physician orders when allowed by Nurse Practice Act.
• Migrate rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
