Job summary

Sweetwater Care in California seeks an experienced LVN in good standing to join our long-term care team as a nurse manager. The role focuses on supervising clinical care within scope, coordinating with physicians, and ensuring adherence to facility policies and guidelines.

The ideal candidate has an active California LVN license, prior nursing management experience, and familiarity with long-term care settings and PointClickCare charting.

Qualifications

• Licensed LVN in California (active)
• Compact license if working in a Compact state
• 1 year nursing management experience preferred
• Long term care experience required
• PointClickCare charting experience preferred

Responsibilities

• Demonstrates knowledge and appropriate use of infection control policies.
• Participates in Quality Improvement program implementation.
• Monitors patient placement using Quality Life Manual.
• Notifies departments of patient changes (rooms, admits, discharges).
• Conducts family and patient education.
• Administers medications and treatments per policy and schedule.
• Receives and transcribes physician orders as permitted by law.
• Makes rounds with physicians when needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
