Job summary

Sweetwater Care is seeking a licensed LVN with nursing management experience to oversee patient care within a long-term care facility. The role emphasizes adherence to policy, timely medication administration, and coordination with physicians to ensure quality outcomes.

The ideal candidate has California LVN licensure (compact license considered for eligible states) and prior long-term care experience, with familiarity in PointClickCare charting and patient education.

Qualifications

• Licensed LVN in California; compact license accepted for other compact states.
• One year nursing management experience preferred.
• Long-term care experience.
• PointClickCare charting experience.

Responsibilities

• Adhere to the Infection Control Manual.
• Participate in the Quality Improvement Program.
• Utilize the Quality Life Manual for patient placement and monitoring.
• Notify departments of patient changes (room changes, LOA, new admits, discharge).
• Provide education to patients and families.
• Administer medications timely and per facility policy.
• Administer treatments timely and per facility policy.
• Receive, transcribe, and carry out physician orders when allowed by the Nurse Practice Act.
• Conduct rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
