Job summary

The People Concern in Los Angeles is offering a temporary Case Manager Apprentice program at The Nest, 222 South Hill St., providing five weeks of hands-on training in permanent housing services. Qualifications

• Valid driver\'s license and reliable vehicle for client transportation.
• Ability to transport clients in personal vehicle.
• Experience engaging with diverse populations.
• Familiarity with basic computer skills and records systems.

Responsibilities

• Collaborate with case management staff to support clients\' independence and health.
• Assist in intake and enrollment of Clients and subsidy applications.
• Support biopsychosocial and functional assessments to guide treatment planning.
• Coordinate appointments, transportation, and follow-up services for Clients.
• Maintain confidentiality and accurate progress notes in electronic databases.
• Participate in weekly trainings, staff meetings, supervision, and evaluations.
