Job summary

Wellnest is seeking an ICMS Case Manager in Los Angeles to join a multidisciplinary team providing service planning and housing support for Transition Age Youth (18-25). The role emphasizes healing-centered, harm reduction, and housing first approaches.

Qualifications

Bachelor's degree in social work or related field required.
Experience with homeless individuals or mental health services preferred.
CA driver’s license and insurable driving record required.

Responsibilities

Formulates individualized service plans addressing housing, health, education and financial needs.
Advocates for clients to obtain housing, healthcare, jobs and benefits.
Provides linkage to health, mental health and substance use services as needed.
Maintains field-based support, transporting participants to resources and appointments.
Documents services in electronic databases (EXYM, HMIS, CHAMP) and attends care coordination meetings.
Represents Wellnest at community events and maintains caseload targets.
