Job summary

The People Concern seeks a dedicated Case Manager, Scattered Site Housing, to support clients in achieving and sustaining permanent housing across the greater Los Angeles area. You will work within a multidisciplinary team to assess housing stability, coordinate medical and psychosocial services, and guide clients toward independence.

You will travel to partner sites, document progress, and maintain confidential records while applying harm-reduction approaches in a fast-paced, on-site

Qualifications

• Bachelor's degree preferred; or two years' case management experience with homelessness.
• Valid CA driver’s license, reliable car, auto insurance, and good driving record.
• Able to transport clients in your vehicle and maintain HIPAA confidentiality.
• Proficient in case management concepts and documentation.
• Experience with harm reduction approaches and coordinating services.

Responsibilities

• Travel to multiple locations in the LA area to assist clients in attaining permanent housing.
• Conduct biopsychosocial assessments and develop individualized case plans.
• Coordinate appointments, transportation, and wrap-around services with partners.
• Maintain up-to-date client files in agency records and databases.
• Document interactions using PIP (Purpose, Intervention, Plan) format.
