Job summary

Brilliant Corners in Los Angeles, CA seeks a Temporary Case Manager to provide intensive case management for 20-30 participants across PSH programs, employing a Housing First, client-centered approach. You will assess needs, connect clients to housing and services, and collaborate with housing, property management and employment teams to promote self-sufficiency.

You will conduct assessments, develop plans, facilitate housing processes, and work in the field across Los Angeles County.

Qualifications

• 2+ years direct mental health and/or intensive case management experience OR 1 year plus a Bachelor’s degree in Social Work, Psychology, Sociology, or related field
• Experience with individuals who have homelessness and mental illness or chronic health issues
• Ability to work in field settings and in community environments

Responsibilities

• Conduct assessments and reassessments to evaluate needs and strengths
• Develop individualized case management plans with clients
• Assist with temporary housing, unit search, lease signing, and housing retention
• Meet clients in home, community, and office; accompany to appointments as needed
• Apply evidence-based practices, including Harm Reduction and Motivational Interviewing
• Provide crisis intervention and connect to 24/7 crisis line
• Link clients to health, benefits, education, employment and other services
• Teach life skills such as budgeting and apartment upkeep
• Coordinate with housing, property management and employment teams
• Build partnerships with community organizations and landlords
• Participate in case conferences to review progress and coordinate care
• Provide transportation assistance including use of personal vehicle when required
• Maintain client files in Salesforce, CHAMP and BOX
• Attend meetings, supervision, and agency events as required
