Job summary

Asthma Virtual Care (AVC) is seeking a proactive coordinator to act as the operational link between patients, digital monitoring tools, and the multidisciplinary care team in Singapore. You will transform patient responses from a chatbot and dashboard alerts into coordinated action, monitor activity, and ensure timely clinical escalation.

You will engage with patients to enrol, document interactions, and drive continuous improvement across operations.

Qualifications

• Diploma or equivalent in healthcare administration, care coordination, nursing support, population health or related discipline.
• Experience in patient-facing healthcare, telehealth, or chronic-disease management is advantageous.
• Familiarity with electronic medical records and multidisciplinary care environments would be beneficial.

Responsibilities

• Outreach to patients to seek enrolment.
• Daily monitoring of the dashboard and reviewing patient responses from the fronting chatbot for routing/escalation to the clinical team per workflow.
• Contact non-responsive patients and guide responses through the chatbot where possible.
• Verify that eligible patients are enrolled and reflected on the dashboard.
• Update and maintain the patient database timely.
• Coordinate appointments, referrals, handovers and communication across patients and clinical teams.
• Ensure accurate, timely documentation of patient interactions, outcomes and escalation across IT systems.
• Collate and validate data including enrolment, response rates, and outcomes.
• Analyse data to identify trends, care gaps, recurring non-response patterns and opportunities to improve operational performance and patient engagement.
• Prepare regular reports and updates, supporting audits and continuous improvement initiatives.
