Job summary

A community care organization in Singapore seeks a dedicated Registered Nurse to provide nursing assessments, develop individualized care plans, and support clients with acute and chronic conditions. The ideal candidate will possess a diploma in nursing, a current registration with the Singapore Nursing Board, and at least three years of clinical experience. Strong interpersonal skills and adaptability are required to work effectively in various home environments.

Qualifications

  • Must be a Registered Nurse (RN) with the Singapore Nursing Board (SNB).
  • Minimum of 3 years of clinical experience in an acute care setting or community/geriatric setting.
  • Valid Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED) certification.

Job description
Roles & Responsibilities

Responsibilities:

Nursing Assessment, developing individual care plan, monitoring and evaluation

  • Assess, monitor, review care needs of client and caregiver.
  • Develop individualized care plan and identify types, duration and frequency of services.
  • Monitor client's response to services and evaluate progress towards goals.
  • Review client's care plan 6 monthly and whenever necessary.
Nursing intervention and treatment plan
  • Schedule times to visit for follow-ups, and remain in contact with the client and / or family throughout the treatment process
  • In carrying out the treatment plan the home care nurse is responsible for the following :
  • Manage acute and chronic medical conditions in consultation with a doctor
  • Perform nursing procedures such as wound care, tube change, medication management and initiate medical care interventions where necessary
  • Provide psychological and spiritual support
  • Provide health education for the client and his caregiver
  • Provide caregiver’s training for the client’s caregiver
  • Maintain updated case records of clients
  • Supervises any delegated task to junior staff
  • Coordinate referrals, provide health and social education
Other Duties
  • Assist and support with administration processes and paperwork when needed
  • Any other general duties as required
Requirements

Qualifications and Registrations:

  • Possess a Diploma or Degree in Nursing.
  • Must hold a valid Practising Certificate and be a Registered Nurse (RN) with the Singapore Nursing Board (SNB).
  • Valid Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED) certification
  • Preferred : Advanced / Post-basic Diploma in a relevant field such as Geriatrics, Community Health, Chronic Disease Management, or Palliative Care.

Experience:

  • Minimum of 3 years of clinical experience in an acute care setting (e.g., medical / surgical ward) or community / geriatric setting is typically preferred.
  • Experience in performing a range of technical nursing procedures independently.
  • Familiarity with the Singapore Community Care sector and resources is an advantage.

Skills and Attributes:

  • Ability to work independently and manage a caseload of patients on home visits.
  • Excellent clinical judgment and critical thinking skills for assessment and intervention.
  • Strong interpersonal and communication skills (written and verbal) to build rapport with patients, caregivers, and external partners.
  • High level of adaptability and resilience to work in various home environments and handle unexpected situations.
  • Proficiency in English and often a second language / dialect (e.g., Mandarin, Malay, or Tamil) to communicate effectively with the local elderly population.
