A community services organization in Singapore seeks a skilled Social Worker to assist clients and families through assessments and counseling. The role involves developing care plans, maintaining accurate case documentation, and collaborating with relevant Ministries and agencies to optimize client support. Candidates should have a degree in Social Work and 2-5 years of healthcare experience, with excellent interpersonal skills to foster relationships with clients and stakeholders.
Assist in conducting assessments of clients and family members in their homes before recommending admission to the Home.
Conduct assessments of severity, key risks, and protective factors for intervention, if necessary, and provide Home Visits, counseling services to help clients and family members manage issues associated with their disabilities, enabling a degree of independent living.
Conduct assessments of the client or family and submit a social report with action plans based on the assessment.
Prepare case history information, psychosocial assessments, and social work treatment plans. Document and maintain case files and prepare reports according to required documentation guidelines and meet Ministry standards.
Conduct closure of cases upon achieving sustainable service outcomes. Plan, develop, and implement curricula, psychoeducational training, and group work for clients and/or their caregivers.
Participate in developmental programs and supervision for personal and professional growth.
Maintain minimum requirements for renewal of membership with the professional association.
Network with other agencies to tap community resources for intervention support to clients.
Work with therapists and training officers in planning individualized care plans.
Assist programme head or supervisor with programme accountability reporting duties.
Evaluate the effectiveness of training and group work interventions.
Held a degree in Social Work from a recognized institution with 2‑5 years of experience in a healthcare setting.
Manage good relationships and communication with various Ministries.
Have excellent communication and interpersonal skills with demonstrated ability to build relationships.
Preferably have a minimum of 3 years of experience in disability work.
Work week: 5.5 days.
