As a Social Worker (Multiple Locations), you will be responsible for the following duties:

Assess the psychological, emotional, social, economic, and physical impact of a patient's medical condition on their lives.

Provide counselling, crisis intervention, and emotional support to patients and their families.

Help patients and families access and utilize community resources, such as financial assistance, housing, transportation, and healthcare services.

Collaborate with other healthcare professionals to develop treatment plans that address the patient's holistic needs.

Provide support and resources for caregivers, recognizing the impact of illness on families.

Assist with discharge planning, ensuring a safe and smooth transition back home or to another care setting.

Maintain accurate and confidential records of their interactions with patients and families.

Requirements:

Possess a Degree in Social Work/ Post Graduate Diploma in Social Work or equivalent.
