Job description

🩺 Job Scope

• Provide compassionate, patient-centred palliative care to patients and families.

• Manage a specialist palliative nursing caseload in hospital and nursing home settings.

• Support end-of-life care, symptom management, and comfort care.

• Work closely with doctors, MDTs, nursing home staff, and caregivers.

• Act as a liaison to ensure smooth transition of care between hospital and nursing homes.

• Guide and support nursing home staff through reviews, discussions, and training.

✅ Requirements

• Min. Diploma in Nursing / State Registered Nurse

• Has Advanced Diploma in Nursing / Palliative Care preferred (for Senior Staff Nurse)

• Relevant nursing experience (palliative / geriatric / nursing home experience is an advantage)

• Good communication and interpersonal skills

• Comfortable working across acute and community settings
