Job summary

A leading healthcare provider in Singapore is searching for a Registered Nurse to join the Primary Care Network team. The role involves providing education on chronic disease and mental health, conducting screenings, and assisting in patient progress management. Ideal candidates will possess strong interpersonal skills and the ability to work dynamically within a team. This position requires travelling across various clinics and offers an opportunity to support doctors in clinical activities.

Qualifications

  • Registered Nurse qualification from the Singapore Nursing Board is essential.
  • Experience in mental health or chronic disease counselling is advantageous.
  • Proficiency in English; knowledge of other languages is a plus.

Job description

Be a part of the Raffles Medical Team group of Primary Care Network (PCN) as we aim to provide quality patient care and counselling through patient education.

Job Responsibilities
  • Provide Nurse Counselling and patient education on Chronic Disease / Mental health management and health promotion.
  • Conducts Diabetic Foot and Eye Screening and providing education.
  • To teach patients’ specific tests for monitoring of chronic disease e.g blood glucose, blood pressure etc.
  • Able to assess patient’s nutritional needs and provide basic nutrition education (Training will be provided)
  • Tracking of patient’s progress in Chronic disease management and lifestyle changes. This can involve face to face consult, tele-consult and telehealth monitoring.
  • Comfortable to conduct talks to small group of patients when need arises.
  • Works closely with PCN Nurse Clinician and Primary Care Coordinators(PCCs) in care coordination to ensure smooth operations of a multifunctional PCN clinic / medical center or clinic cluster.
  • Participate in PCN meetings, CME activities and case discussions.
  • When needed, function as member of the frontline clinical workforce and support doctors in various clinical activities.
  • Undertake any other responsibilities as delegated by the direct supervisor, director or medical director.
Job Requirements
  • Registered Nurse with the Singapore Nursing Board
  • Prior experience in mental health / chronic disease counselling will be advantageous
  • Good interpersonal skills with a pleasant personality
  • Enjoy working with people
  • Able to work in a dynamic environment as well as being flexible and open to changes.
  • Must be willing to travel to different locations across RMG Clinics for PCN services.
  • Proficient in English and knowledge of other languages or dialects will be advantageous.
