Job summary

A healthcare provider in Singapore is looking for a Registered Nurse to coordinate diabetic foot screening services and provide patient education on Healthier SG. The ideal candidate will have a diploma or degree in Nursing and be registered with the Singapore Nursing Board. They should possess good interpersonal and communication skills, be fluent in English, Chinese, and a local dialect, and have experience in mental health. This role also involves working closely with general practitioners and ensuring quality patient care.

Qualifications

  • Experience in mental health and chronic disease management will be an advantage.
  • Experience in healthcare or social services.

Responsibilities

  • Coordinate and Schedule Diabetic Foot Screening Services and Nurse Counselling Services.
  • Assist in implementation and educating patients on Healthier SG.
  • Work closely with GPs to manage identified patients their community services.
  • Monitor the quality of care provided by GPs and primary care professionals.
  • Assist in data collection for quality improvement projects.

Job description
Job Responsibilities
  • Coordinate and Schedule Diabetic Foot Screening Services, Diabetic Retinal Photography and Nurse Counselling Services
  • Assist in implementation and educating patients on Healthier SG and benefits
  • To work closely with the GPs to manage identified patients who need support and referral to community services
  • Tracks, Monitors and follows up on the quality of care provided by GPs and other Primary care professionals and to ensure maintenance of good clinical care
  • Assists GPs in extraction and submission of data and other relevant administrative work
  • Involve in the PCN planning team to ensure smooth coordination within PCN
  • Has experience conducting screening and provide comprehensive health education to patients with chronic conditions
  • Ability to assess patient’s medication compliance, social history, current lifestyle and dietary habits
  • Has the ability to provide good clinical judgement, provide patient care in line with work instructions and protocols
  • Assist in data collection for quality improvement projects
  • Willing to work on Saturdays when required
  • Perform any other task assigned by PCN leader or Supervisor
Job Requirements
  • Diploma or degree in Nursing
  • Registered Nurse with the Singapore Nursing Board
  • Experience in mental health and chronic disease management will be an advantage

  • Has a positive attitude and a good team player
