Job summary

A prominent healthcare facility in Singapore is seeking a qualified Social Worker to provide holistic case management support and facilitate group work sessions. The ideal candidate will possess a Degree in Social Work and have strong communication skills. Responsibilities include community networking, program development, and supervising junior staff. Proficiency in Microsoft Office and the ability to speak dialects such as Malay or Mandarin are preferred. Qualifications

  • Fresh graduates must have completed a minimum of 400 supervised placement hours.
  • Staff with experience should be eligible for accreditation with the Social Work Accreditation Board (SWAB) or be a Registered Social Worker.

Responsibilities

Job description
Responsibilities
  • Provide holistic case management support and appropriate level of care across the continuum of care services within the campus and into the community through assessment, care planning, interventions and evaluation of care.
  • Member of Quality improvement related projects that will improve patient’s outcome.
  • Staff will need to meet department clinical indicators.
Group Work
  • Assist in developing, conducting and facilitating group work sessions to establish social support systems and networks for target groups
  • Engage individuals and/or target groups who may benefit from group work.
Community Work
  • Network and collaborate with community partners to conduct needs assessment and implement programs to enhance the psychosocial wellbeing of individual patients and community
Programme Development and Implementation
  • Participate, implement and assist in the development of programs to improve the quality of care and services.
Education / Clinical supervision
  • Provide guidance, training and co-supervision to social work assistant, senior social work assistant and student medical social worker in their standards of practice delivery.
  • Participate in developmental programs, learning opportunities, and supervision for personal and professional growth and development.
  • Conduct psychoeducational talks to patients/public with guidance as needed.
  • Assist to conduct professional workshops and seminars.
  • Assist to organize, coordinate and provide continuing education within Woodlands Health.
Research
  • Apply and demonstrate evidence-based practice.
  • Critically evaluate current research literature and share with others
  • Participate and support research data collection/design/analysis
  • Assist to develop abstracts/posters/oral presentations
Management and Leadership
  • Assist to manage services and resources to achieve targets set by department/ Woodlands Health
  • Assist in supporting department’s workgroup to deliver and improve service and care
General
  • To apply social work ethics and values, legislation and international conventions to practice
  • To apply strategies and be instrumental to advocate for beneficiaries
  • To understand socio-economic-political system within a multi-cultural context and relevant policies
  • Actively involved in department and Woodlands Health’ initiatives
  • Any other duties assigned by Supervisor/ Head of department
JOB REQUIREMENTS
Education / Training / Experience
  • Degree/ Post Graduate Diploma in Social Work
  • For fresh graduate: To have completed minimally 400 of supervised placement hours.
