Job summary

Ren Ci Hospital is seeking a healthcare administrator to manage referrals and provide end-to-end case management for home care clients requiring Home Personal Care, Home Nursing, Home Therapy, Home Medical and Medical Escort services. The role supports admission processes and coordinates service delivery with a multi-disciplinary team.

The position emphasizes client-oriented service, data accuracy, and efficient process handling, with weekend standby availability and strong MS Office proficiency.

Qualifications

• Diploma in Nursing / Allied Health / Psychology / Counselling / Social Work / Sociology/ Community & Social Service (Eldercare) preferred
• At least 4 years of working experience
• Preferably 1 year of ILTC sector experience
• Proficient in MS Word, Excel, PowerPoint and Outlook
• Must be client-oriented, self-directed, independent yet able to work in a multi-disciplinary team with good interpersonal and communication skills
• Excellent time management, well organised and outcome-focused
• Able to be on standby calls on weekends

Responsibilities

• Manage home care client data base to ensure completeness and accuracy
• Process home care referrals through AIC referral system as liaison
• Perform basic financial assessment for clients or next-of-kin and refer to social worker for assistance
• Perform basic assessment of care needs and conduct home visits when required
• Coordinate with volunteers to engage home care clients meaningfully
• Participate in multi-disciplinary discussions to address social and clinical needs
• Respond to complaints and feedback from clients and next-of-kin
• Manage and schedule deployment of care staff based on preferred date/time/frequency
• Apply for Senior Mobility Fund and process invoices monthly
• Check and verify invoices for services rendered monthly
• Submit data to ILTC and finance for invoice processing monthly
• Collect data for reporting to management and regulatory authorities
• Participate in projects to improve quality of care and services
• Identify service gaps and recommend improvements
• Any other duties as assigned by reporting officer

Job description

Provide end-to-end case management support and coordination to ensure a smooth admission process and service delivery for home care clients that require Home Personal Care, Home Nursing, Home Therapy, Home Medical and Medical Escort services, etc.

Client Management

• Manage home care client data base to ensure completeness and accuracy
• Process home care referrals through AIC referral system as point of liaison
• Perform basic financial assessment for clients or next-of-kin on home care service(s) required and refer to social worker if financial assistance is required
• Perform basic assessment of care needs for home care clients and conduct home visits where required
• Coordinate with volunteers to engage home care clients meaningfully
• Coordinate and participate in multi-disciplinary discussions to address the social and clinical needs of the clients
• Respond to complaints and feedbacks from clients and next-of-kin

Administrative duties

• Manage and schedule deployment of care staff for clients based on their preferred date, time and frequency of service, etc
• Apply for Senior Mobility Fund for clients and process the invoices every month for claims and payment
• Check and verify invoices based on the services rendered every month
• Submit relevant data to ILTC and finance department for processing of invoices every month
• Collect relevant and accurate data for reporting to management and regulatory authorities
• Participate in project work to improve the quality of care and services to patients
• Identify service gaps in response to clients’ needs and make recommendations to improve service provision of home care
• Any other duties as assigned by reporting officer.

Requirements:

• At least a Diploma in Nursing / Allied Health / Psychology / Counselling /Social Work / Sociology/ Community & Social Service (Eldercare) is preferred
• At least 4 years of working experience
• Preferably 1 year of working experience in ILTC sector
