Job summary

Ren Ci Hospital is seeking a dedicated professional to manage referrals and coordinate home care services including personal care, nursing, therapy, and medical escort. You will handle client data, referrals, and invoicing while ensuring timely service delivery and collaboration with a multi-disciplinary team.

The role requires a Diploma in Nursing/Allied Health, at least 3 years in ILTC, and strong MS Office skills, with emphasis on client focus and time management.

Qualifications

• Diploma in Nursing / Allied Health preferred.
• At least 3 years of ILTC sector experience.
• Able to use local languages and dialects.
• Proficient in MS Office (Word, Excel, PowerPoint, Outlook).
• Client-oriented, self-directed, independent yet able to work in a multi-disciplinary team and have good interpersonal and communication skills.
• Excellent time management, well organized and outcome focused.

Responsibilities

• Manage home care client database ensuring completeness and accuracy.
• Process home care referrals through ILTC referral system.
• Perform basic financial assessment for clients or next-of-kin and refer to social worker if needed.
• Conduct home visits to assess care needs.
• Coordinate referrals of services (home personal care, nursing, social work).
• Coordinate with volunteers to engage home care clients.
• Respond to complaints and feedback from clients and next-of-kin.
• Schedule deployment of care staff per client needs and frequency.
• Apply for Senior Mobility Fund and process monthly invoices for claims.
• Check and verify invoices monthly and submit data to ILTC and finance for processing.
• Collect data for reporting to management and regulatory authorities.
• Participate in quality improvement projects.
