Diaverum AB is seeking a dedicated Social Worker to plan, coordinate, deliver and evaluate social care services across assigned Saudi clinics. You will provide psychosocial support, health promotion, and patient education, working closely with nurses, physicians, and management to enhance care quality and patient well-being.

You will implement a continuous social care process, maintain comprehensive documentation, and participate in quality improvement initiatives.

Qualifications

• Bachelor’s degree in Social Work is required; a Master’s is favourable.
• Certified Clinical Social Worker or equivalent credential is desirable.
• Fluent in Arabic and English, both spoken and written.
• Proficient in MS Office and basic computer systems.

Responsabilités

• Provide psychosocial support and health promotion for dialysis patients.
• Implement the Social Care Process: screening, assessment, intervention, and counselling.
• Coordinate patient support programmes for social services, financial aid and transportation.
• Document Monthly Care Plans, follow-up records and referrals accurately.
• Ensure compliance with social care policies, regulatory requirements and quality standards.
• Participate in social care research, training and quality improvement initiatives.
