Qualifications

• Able to work weekends and support outreach or screening events.
• Previous healthcare, community care, customer service, operations or event coordination experience is an advantage.

Responsibilities

• Coordinate and schedule frailty screening and follow-up appointments for residents.
• Contact residents by phone and SMS for bookings, reminders, and follow-ups.
• Educate seniors and encourage participation in frailty screening and health promotion.
• Manage screening records, participant databases, appointment tracking, and reports.
• Handle enquiries from residents, partners, and healthcare stakeholders.
• Support community outreach activities, health talks, roadshows, and onsite screening.
• Liaise with providers and vendors to ensure smooth programme delivery and follow-up care.
• Facilitate referrals and follow-up actions for further assessment or intervention.
