The Senior Executive / Assistant Manager, Care Transition Services (CTS), supports the planning, operations, implementation, and management of care transition programmes across NHG Health hospitals, primary care and the community care sector.
The role supports the CTS satellite offices within hospitals by working closely with specialist departments and hospital operations teams to drive right-siting initiatives, review discharge processes, and improve care transition workflows. This includes partnering with Right-Siting Officers (RSOs) and clinical teams to facilitate the timely and appropriate transition of patients from hospitals to primary care and community care providers.
The incumbent also supports the development of integrated care pathways and operational processes that enable seamless patient flow between care settings. Working closely with Clinical Liaisons, hospital teams, and Population Health units, the role helps strengthen care integration and connects patients to relevant community and primary care resources to support continuity of care.
Perform administrative and operational duties to support assigned services, programmes, and workstreams.
Ensure the smooth day-to-day operations of assigned programmes and initiatives.
Collaborate with clinical departments, hospital operations teams, and community partners to implement and sustain care transition initiatives.
Coordinate with stakeholders to ensure discharge planning resources, care pathways, and workflows are effectively communicated and implemented.
Maintain and strengthen relationships with internal and external partners across the continuum of care.
Track, monitor, and maintain programme data and performance indicators.
Prepare reports, presentations, dashboards, and data analyses to support operational and strategic decision-making.
Support the development, updating, and maintenance of discharge planning resources and operational guidelines.
Draft papers, proposals, reports, and other documentation as required.
Support supervisors and Clinical Liaisons in the planning, implementation, and management of projects related to care integration, clinical pathways, and operational process improvement.
Coordinate project activities, stakeholder engagements, and implementation plans to ensure timely delivery of project outcomes.
Monitor project progress, risks, timelines, and budgets, and provide regular updates to management.
Facilitate communication and change management efforts to support the successful implementation of new initiatives and workflows.
Monitor programme performance and support the achievement of operational, service utilisation, and financial targets.
Support budget planning and ensure programmes operate within approved budgets.
Ensure compliance with service standards, operational requirements, institutional policies, and relevant regulations.
Maintain departmental tracking and monitoring systems and ensure data integrity.
Identify opportunities for process improvement and support the planning and implementation of initiatives to enhance service quality, efficiency, and patient outcomes.
Provide secretariat support for departmental, divisional, and cross-functional meetings.
Plan, coordinate, and manage projects, events, and institutional initiatives as assigned.
Support onboarding, orientation, and training of new staff.
Build and maintain effective working relationships with internal and external stakeholders.
Undertake any other duties and responsibilities as assigned by the supervisor.

Relevant/related working experience in community care/ communications/ project management/ business will have advantage.
Possess good interpersonal and strong communication (verbal and written) skills.
Familiar with MS Office applications.
